I don't want anyone to have to be burdened with looking after me if I was a "vegetable" Much rather have people remember me as I was and be able to grieve properly rather than eventually resenting me for not being alive but not being dead either.

I think its a great idea but I think in fact it won't matter...much like a Donor Card.  If you don't have any spouse, siblings or next of kin then great.  But if you do, the ultimate decision will lie with them.  A donor card is known as an "expression of wish" nothing more.

I also believe that sometimes things can happen that would contravine such a wish.  For instance, I recently was personally involved with my sister whose child had a serious accident and lay on life support in ICU.  If they had decided to turn the machine off due to the trauma to his head, from which they said his brain stem was completely dead, he would have died.  However, they gave it a few days and 18 months later he is fighting fit (not 100% but everyday is step in the right direction) and goes to school.

However, in the case of elderly or terminally ill patients this is different again.  Hence why I think that are too many variables that can add to the situation which would go against any such expression of wish.
